The Intake Coordinator is responsible for the admission process for new patients by ensuring availability of reimbursement for services, patient intake, identifying the clinical team to deliver care, and scheduling.

Responsibilities

LPN/RN Preferred

  • Complete the intake process from initial referral contact to home health admission (must be proficient with ERM and various online-based software)
  • Provide direct oversight of patient education and preparation for home care services, plan of care initiation, and coordination of care with multiple service providers, this includes verifying patient-physician information and completing patient intake
  • Utilize clinical knowledge to triage incoming referrals to determine the necessity of timeline initiation of care based on regulatory guidelines regarding initiation of care.
  • Communicate directly with patients and families to ensure acceptance of services and coordinate the insurance verification and authorization processes
  • Document pertinent information regarding the patient’s demographics, medical condition, medications and treatments, and service being requested
  • Communicate home care criteria to referral sources and serve as a resource for home care criteria and alternatives to internal and external customers
  • Ensure face-to-face visit has been arranged if needed, including follow-up MD appointments
  • Establish and maintain positive working relationships with current and potential referral sources
  • Ensure compliance with all state and federal legal and regulatory requirements
  • LPN/ Registered Nurse with current licensure to practice in the state of operation (PA) - Preferred
  • 2 years healthcare experience, home care experience preferred
  • Working knowledge of insurance reimbursement guidelines
